- Medical Invitation Letter – From registered hospital in China (Kunming Tongren)
- Medical Report / Doctor’s Prescription
- Photograph – Recent passport-size photo
- NID or Birth Certificate
- Air Ticket – Confirmed or tentative
- Hotel or Hospital Booking Confirmation
- Bank Statement – Minimum balance 10 lakh BDT (last 6 months updated)
- Bank Solvency Certificate
- Visa Application Form – Filled and signed
- Travel Health Insurance (recommended)
- Consent Letter – For minor patient
Attendance/Accompanying Person Documents
- Passport – Minimum 6 months validity
- Photograph – Recent passport-size photo
- Relationship Proof – NID/Birth Certificate/Family Relation Certificate
- Bank Statement & Solvency Certificate – Same as main applicant
- Hotel or Hospital Booking – Showing attendant name (if possible)
- Invitation Letter – Hospital/Doctor note mentioning attendant name
